What is required of a vendor or supplier that I want to have added to the University Supplier Database?

Send us the following information on your desired vendor:

• Full legal entity name
• Contact First and Last Name (best if the contact has financial information at their disposal for completing the application process)
• e-Mail address for the Contact
• Physical Address for the Vendor
• A brief description of the goods and/or services that this vendor will be providing
• Is the supplier a foreign person or entity?
• If the supplier is foreign, will the services be performed in the United States?
• Any special or additional comments you would like the Supplier Data Management Team to be aware of in regards to this vendor?
